1. How Can I Request Copies of My Medical Records?
Under federal and Texas law, you have the right to request access to your protected health information (PHI) maintained by this practice. You may request medical records for personal use or transfer to another provider by submitting a written request. We generally must provide your records within 15 business days after we receive your request and any applicable authorization or fees.
To request records:
• In your Cerbo portal, complete our Medical Records Request Form (or ask our office for a copy).
• Submit the form by portal, mail, email, fax, or in person.
• We will review your request and provide your records in electronic or paper form as requested, unless legally permitted to deny access with appropriate notification.
If access is denied for health or legal reasons, you will be provided a written explanation and information on how to appeal or file a complaint.
(Note: We may charge a reasonable fee for providing copies as allowed by law, which may include copying and postage or electronic delivery costs.)
2. How Do I Correct or Amend My Medical Records?
You have the right to ask that your health record be corrected or amended if you believe information is incomplete or incorrect. Submit your request in writing through the secure Cerbo portal, specifying the amendment you seek. If we disagree, we will provide a written denial with reasons and describe how you can submit a statement of disagreement to be maintained in your record.
3. What Are My Privacy Rights?
We respect your privacy and handle your protected health information (PHI) in accordance with the federal HIPAA Privacy Rule and the Texas Medical Privacy Act. You have rights under these laws, including:
• The right to receive a Notice of Privacy Practices explaining how your information is used and disclosed. As a patient or our practice, you can review your signed HIPAA privacy rights disclosures in the Cerbo portal by going to Labs and documents, and viewing either your Registration form, or your HIPAA privacy Authorization form .
• The right to request restrictions on certain uses and disclosures.
• The right to receive confidential communications.
• The right to an accounting of certain disclosures of your PHI.
If you believe your PHI has been improperly used or disclosed, you may file a complaint with our office, the U.S. Department of Health and Human Services (Office for Civil Rights), or the Texas Attorney General’s Office.
4. How Can I Contact the Texas Medical Board (TMB) or File a Complaint?
If you have concerns regarding professional conduct, licensure, or compliance with medical practice standards, you may file a complaint with the Texas Medical Board (TMB):

Please note that anonymous complaints generally cannot be investigated by the TMB; the Board will evaluate each submission within its jurisdiction.
5. How Can I File a Complaint with Texas Health and Human Services (HHS)?
You may file a complaint about a health care facility, regulated provider, or violation involving protected health information or provider compliance with Texas HHS regulations by contacting:

6. How Do I File a Federal HIPAA Privacy Complaint?
If you believe your HIPAA privacy rights have been violated, you also have the right to file a complaint with the U.S. Department of Health and Human Services (HHS), Office for Civil Rights:
